GLP-1 Medications and Polycystic Ovary Syndrome (PCOS)

What is PCOS and insulin resistance?

Polycystic Ovary Syndrome (PCOS) is a common endocrine disorder, affecting many women of reproductive age. It shows up differently for everyone, but common symptoms include:

  • Irregular or missed periods
  • Weight gain or difficulty losing weight
  • Unwanted hair growth (face, chest, or back)
  • Oily skin and acne
  • Fertility struggles

A key driver of these symptoms is insulin resistance. This happens when the body does not respond properly to insulin, the hormone that controls blood sugar. The body then produces more insulin, which throws hormones off balance and can worsen PCOS symptoms.

Up to 70% of women with PCOS have insulin resistance, which is why improving insulin sensitivity is a core part of treatment.

GLP-1 medications and PCOS: how they work together

GLP-1 receptor agonists (GLP-1s) such as Ozempic, Wegovy, and Mounjaro were originally developed for type 2 diabetes. But research shows they can also improve insulin sensitivity, supporting hormone balance, weight loss, and reproductive health.

How GLP-1s may help with PCOS:

  • Weight loss: Reduce appetite and slow digestion
  • Hormone regulation: Lower androgen levels, which can ease acne, hair growth, and irregular periods
  • Improved menstrual cycles: Many women report more regular periods and ovulation
  • Fertility support: Improved chances of conception
  • Better blood sugar control: Reduced risk of prediabetes and type 2 diabetes

In clinical trials, GLP-1s have been shown to restore regular bleeding cycles in more women than placebo and are often more effective than metformin at reducing insulin resistance and lowering androgen levels.

The SheMed view

For women with PCOS, GLP-1 medications may offer support beyond weight loss. By improving insulin sensitivity, they can help balance hormones, restore cycles, and reduce long-term health risks.
