Mounjaro to Wegovy Dose Conversion: What's the Equivalent and Where Do You Start?

So you're considering changing from Mounjaro to Wegovy (or maybe vice versa) and the first thing you google is: "what dose do I start on?" That’s completely valid. If you're working hard to get to your current dose, the last thing you can afford is to lose is momentum in a bad transition.

The good news is that switching doesn’t mean you start anew. But it’s something to know a bit about.

Here’s the straight up answer: between Mounjaro and Wegovy, no official, published dose conversion chart exists. These are two distinct medicines whose functions differ, so you can’t just pick one drug as replacement.

That doesn’t mean you have to switch with a lot of anxiety or risk. It means that not only does your prescriber have to figure out for you an actual clinical plan, but instead of the formula, a structured one has to be created.

We will take you through what the evidence tells us, what your likely starting points will look like and just what your clinician is going to be thinking when they decide that the right dose for your switch will be.

Key Takeaways

  • There is no officially published Mounjaro to Wegovy dose conversion chart.
  • Doses can’t be directly compared, as the two medications behave differently in the body.
  • Most patients transitioning from Mounjaro to Wegovy will use an equivalent dose of Wegovy
  • There is usually a one week washout period while switching
  • Your prescriber can consider your clinical history, current dose and tolerance to choose the safest potential place to start.
  • Our clinical team at SheMed makes each dose decision, and you can always leave it up to them without having to decide on your own.

Why Do People Switch from Mounjaro to Wegovy?

It’s never one single answer, and knowing your “why” is what allows you to get the transition right mindset.

These are the most common reasons why women switch from Mounjaro to Wegovy:

  • Supply issues. In the UK, Mounjaro has had availability issues in the past, which resulted in many women switching medications to maintain continuity of treatment.
  • Provider changes. If you are transferring to a new provider, such as SheMed, and they’re prescribing Wegovy as part of their programme, a managed switch is often the most efficient and simple way to make this switch without stalling things.
  • Side effects on Mounjaro. Other women believe that Mounjaro’s dual-hormone action is excessive, particularly at higher doses. Chronic nausea, tiredness or GI discomfort that does not resolve can be an indication that Wegovy’s single-hormone mechanism may be more appropriate for your body.
  • Clinical recommendation. Your prescriber would likely recommend Wegovy given your own health profile (especially given your particular cardiovascular considerations, since Wegovy (semaglutide) is also available for licensed use in reducing the risk of heart attack and stroke in eligible patients).
  • Cost and access. Prices can be extremely different depending on medications and providers. Women may find it easier to afford Wegovy since they are already at the dose-point or are more able to access it through their chosen programme.

None of those things is evidence that Mounjaro wasn’t working for you. They’re practical realities of navigating a treatment landscape that is still in flux. Ultimately, the need to make that transition is to do it safely, with appropriate clinical support and without this unnecessary opening in treatment.

Why There’s No Direct Mounjaro to Wegovy Dose Conversion

Mounjaro (tirzepatide) and Wegovy (semaglutide) are both weekly injections that aid weight loss, but they do so using different mechanisms:

  • Wegovy (semaglutide) replicates one hormone, GLP-1, that supports appetite and blood sugar regulation.
  • Mounjaro (tirzepatide) is a synthetic agonist of two essential hormones GLP-1 and GIP, therefore generally exerts a greater overall effect on appetite and metabolic function.

Mounjaro works in an additional hormone pathway, making it a more potent medication per milligram. It won’t really make sense to look at the numbers on the pen and think they correspond to the same amounts. The dose of 5mg Mounjaro is not a 5mg equivalent of Wegovy. The units, ranges and mechanisms are all different.

To understand this better - let’s say you’re used to driving something with a super-responsive throttle and now you shift to a less responsive throttle-based car. If you press the pedal the same way, you won’t get the same speed. Doesn’t mean the car is worse. It just operates differently, and you need to give yourself time to get used to it.

(The UK Medicines regulator (MHRA) has confirmed in its 2026 recommendation that the only person making fluid transition through the spectrum of GLP-1 medicines will be your prescriber.)

Mounjaro to Wegovy Dose Conversion: What the Evidence Says

It lacks a peer-reviewed conversion guide to refer to specifically because there have been no direct clinical trials comparing Mounjaro doses to Wegovy doses in a switching context. But what they do have is clinical experience among prescribers who have successfully performed thousands of switches, and efficacy data from the different SURMOUNT-1 (Mounjaro) and STEP UP (Wegovy) trials.

Everything we know from the trial data:

  • Mounjaro (15mg) delivered an average weight loss of 22.5% of body weight at 72 weeks (SURMOUNT-1).
  • Wegovy at maximum dose (2.4mg) resulted in a mean weight loss of 20.7% body weight at 72 weeks (STEP UP).

Both are highly effective. Mounjaro, for example, has an average result a little higher than the other option, driven by its dual-hormone action.

At SheMed, we usually prescribe an equivalent dose while switching from Mounjaro to Wegovy, unless the patient prefers starting on a lower dose. The progress is then monitored with expert supervision to ensure the transition is smooth.

Wegovy Dose Equivalents: How Each Mounjaro Dose Compares

The table below is a clinical reference guide and not a prescription. It reflects the kind of starting doses prescribers commonly consider when switching from Mounjaro to Wegovy, based on clinical experience and individual assessment. Your actual starting dose may differ.

Wegovy dose equivalents table explanation
Important: The dose guidance in these tables is for reference only and is not a substitute for clinical advice. There is no officially published conversion chart between Mounjaro and Wegovy. Your prescriber will always determine the right starting dose for you based on your individual history and health. Never adjust your dose without clinical approval.

5mg Mounjaro Equivalent to Wegovy: What to Expect

If you’ve been on 5mg Mounjaro, which is typically the first or second titration step, then it is already in the earlier treatment stages. Changing from here generally means starting Wegovy at 0.25mg or 0.5mg, which is Wegovy’s introductory dose range. This is intentional. Either pharmaceutical method implements a “start low, go slow” strategy to allow your body to acclimatise.

Nausea and other nasty side effects are largely common at the beginning, and starting at the lowest Wegovy dose substantially reduces that risk. The first 4–8 weeks should feel like a reset process, much like the 1st time you used Mounjaro. Loss of appetite will be present, but weight loss usually persists.

7.5mg Mounjaro to Wegovy: Dose Guidance

7.5mg Mounjaro to Wegovy: Dose Guidance. Seven and a half milligrams of Mounjaro is a mid-range dose where many people start to see stronger appetite suppression and consistent weight loss. When switching from this level to Wegovy, your provider suggests starting at 0.5mg to 1mg.

You might find that appetite control looks a bit different on Wegovy, and that’s because you’re losing GIP hormone function that Mounjaro provided. Others take weeks to get into their rhythm; some people learn quickly. That’s totally normal, and your clinician can then adapt from there.

10mg Mounjaro Equivalent to Wegovy

At 10mg Mounjaro, you are well into the treatment programme and are probably already feeling the improvements. Switching here generally means that Wegovy commences at 1mg, the middle of Wegovy’s dose range.

1mg Wegovy is a dose that most people tolerate and has a clear evidence base for weight reduction. From here, your prescriber will titrate you depending on how you react, the intent being to achieve your maintenance dose over a number of weeks.

Mounjaro Wegovy Equivalent Doses: The Full Comparison

Looking at the full dose ranges side by side helps to understand the scale difference between the two medications:

  • Mounjaro dose range: 2.5mg → 5mg → 7.5mg → 10mg → 12.5mg → 15mg
  • Wegovy dose range: 0.25mg → 0.5mg → 1mg → 1.7mg → 2.4mg → 7mg (exceptional cases)

The numbers may look very different, but that doesn’t mean one drug is stronger than another at all times. This dual-hormone action of Mounjaro means that smaller milligram concentrations can yield dramatic effects. The single-hormone basis of Wegovy's therapy is successful at the higher doses.

This is precisely why a numeric comparison does not work when it is done directly, and why clinical judgement is also key.

What is 1mg Wegovy equivalent to in Mounjaro?

1mg Wegovy is commonly accepted as a moderate maintenance dose within the midrange of Wegovy’s titration programme. Clinically speaking, the drug of choice for patients transitioning from Mounjaro doses in the range of 7.5 to 10mg is 1mg Wegovy; it is commonly used as a starting or step-up dose.

Here’s why:

·       At this time on Mounjaro, appetite suppression is well established.

·       1mg Wegovy delivers significant GLP-1 activity but does not jump to peak strength.

·       There is room for titration up to 1.7mg or 2.4mg if needed, giving your clinician flexibility.

But people who are going between 10mg or 12.5mg Mounjaro may start on 1.7mg Wegovy, especially if they've been on a high dose for a while and have been tolerating this well. Your prescriber will assess your entire picture, not the numbers.

What is 2.4mg Wegovy Equivalent to in Mounjaro?

2.4mg Wegovy is approximately the same value as 12.5mg to 15mg Mounjaro, which is closer to the upper range of each medication.

If you’re on 15mg Mounjaro and converting to Wegovy 1.7mg, your prescriber may try to titrate you up to 2.4mg the next month if you are able to tolerate dose and if you wish

Most switchers aim to hit 2.4 mg of Wegovy. For the general patient, this is a place where there is the best chance of long-term effect.

It’s worth managing expectations here: whether you were taking 15mg Mounjaro or not, you wouldn't be starting Wegovy at 2.4mg. The titration procedure continues from now on because Wegovy is a completely new drug in your body no matter how long you have used Mounjaro.

Interesting fact: A 7.4 mg dose has recently (Jan, 2026) been approved in the UK (the first nation in the world to greenlight this high-dose regimen). Recent studies (Novo Nordisk's STEP UP trial program) indicate that patients on this dose have been able to reduce 20.7% to 21% of their total body weight over 72 weeks.

Wegovy to Mounjaro Conversion: Switching in the Other Direction

The same principles are still true, only in reverse.

The first thing most people notice is that switching from Wegovy to Mounjaro tends to feel stronger, even at lower doses. That's because Mounjaro adds a second hormone pathway (GIP) on top of the GLP-1 action you're already used to from Wegovy. Your body hasn't encountered GIP before, so it's a new experience for your body.

If Wegovy has delivered solid results for you, but weight loss has started to plateau, this dual-action mechanism is often what prompts the switch. Mounjaro can sometimes restart progress where semaglutide alone has reached its ceiling for that individual.

Timing your switch: the washout question

Semaglutide has a half-life of approximately seven days. This means a gap of one week between your last Wegovy dose and your first Mounjaro dose is generally well-tolerated. So basically, you can take your last Wegovy injection, wait one week, and begin Mounjaro. Your prescriber will confirm the exact timing for your situation.

Wegovy to Mounjaro Conversion Chart: How It Works

Regardless of the Wegovy dose you were on, it is not generally recommended to start Tirzepatide (dual-action) at anything above 5mg when switching from Semaglutide (single-action).

This isn't about how well Wegovy was working. It's about the fact that Mounjaro's GIP action is entirely new to your body, and combining a new hormone pathway with a higher starting dose significantly increases the risk of side effects like nausea, vomiting, and GI discomfort in particular.

The table below shows the usual Mounjaro starting doses for people switching from Wegovy. So, like the one above, this is not a prescription but clinical guidance.

Important: Important: The dose guidance in this table is for reference only and is not a substitute for clinical advice. There is no officially published conversion chart between Wegovy and Mounjaro. Your prescriber will always determine the right starting dose based on your individual history and health. Never adjust your dose without clinical approval.

One exception worth noting: a clinician may decide that a patient who has been stable on a higher Wegovy dose for a meaningful period of time is appropriate to start at 5mg Tirzepatide rather than 2.5mg. This is, however,  an individual clinical decision and it would always be made by your prescriber based on your specific circumstances.

What Mounjaro Dose After Wegovy 2.4mg?

(We know this question may sound grammatically wrong, but we just had to stick to the exact framing of how people are asking questions on the internet.)

Moving from 2.4mg Wegovy, the peak dose available (until Jan 2026), to Mounjaro, tells your prescriber a lot about where you are in your treatment journey.

Even coming from 2.4mg Wegovy, the recommended starting dose for Mounjaro is 2.5mg. Your prescriber may, however, decide if 5mg is appropriate based on how well-stabilised you were on 2.4mg Wegovy for a meaningful period.

Here's why it’s for your own good -

  • Mounjaro works via two hormones simultaneously, as against the single hormone action of Wegovy. So even a “little” dose can feel pretty powerful at first.
  • Starting low lowers nausea/vomiting and GI side effects in the adjustment period

For the vast majority of people, titrating from 5mg to 10mg to 15mg Mounjaro following Wegovy 2.4mg is also a relatively smooth transition, often faster than the first time round, as your physiology is already on high alert to GLP-1 action. But everyone feels different and your clinician will dictate how you feel.

How Your Prescriber Decides Your Starting Dose When Switching

When a prescriber recommends a first dose, they aren’t guessing. Each time you switch, there’s a structured clinical assessment as the basis. Here’s what they’ll consider:

  • What dose you currently take and how long have you been on it. The older and more established your dose is, the more information your prescriber has about your tolerance.
  • How well you’ve been taking your current medication. If you’ve experienced major side effects, they’ll begin you even more cautiously on the new medication.
  • Your weight loss progress so far. If you have been losing steadily, they want to protect that momentum as you go through the transition.
  • The gap between the last dose you took and the new one. Switching between Mounjaro and Wegovy does not involve washout, but the timing of your last injection is important for continuity.
  • Your overall health picture. Blood test results, metabolic markers and any co-existing conditions are all input in that decision.

Which is why switching shouldn’t be something that you try to work out yourself based on whatever you’ve read online on it. The dose guidance in this article does provide a helpful frame of reference, but your prescriber can provide the context you can’t find in a search engine.

Mounjaro to Wegovy Dose Conversion at SheMed: How It Works

At SheMed, we make sure your transition is carefully planned for you and not just using a simple formula. Here is what it’s like when you switch over to SheMed:

  • Blood test included from day one. Each new SheMed member undergoes an at-home blood test. This provides our clinical team with full insight into your metabolic status such as thyroid function, liver enzymes, HbA1c and cholesterol, before we prescribe anything. Nearly all providers bypass this altogether.
  • Clinician-led dose review. Before recommending where to begin, our clinical team looks over your history, your previous dose, and overall health. We do consider your personal preferences, however, our recommendation would always be guided by science.
  • Side-effect care pack included. Switching medication can sometimes mean a temporary adjustment to another drug, nausea being the most common adjustment. We offer a side-effect care pack that is included in your first month to help ease the process.
  • Ongoing 6 and 12-month blood tests. We’re not a ‘prescribe and disappear’ service. Our programme also involves repeat blood testing at 6 and 12 months, so that we can assess how the medication is affecting your metabolic markers, and not just your weight.
  • Transparent pricing from £59/month. The initial month covers your blood test, side effect care pack and full clinical support. No hidden fees.

Frequently Asked Questions

Is there an official Mounjaro to Wegovy dose conversion chart?

No, and it’s okay. There is no officially published conversion guide because Mounjaro and Wegovy work via different mechanisms and they have different dose ranges. Your prescriber will decide what the appropriate initial dose to start with is for you, based on your unique clinical picture.

Do I need a washout period when switching from Mounjaro to Wegovy?

No. Because you’ve been tolerating your current medication well, you can take your last Mounjaro dose one week and switch to Wegovy 7 days later. Your prescriber will have the timing at the exact point for you.

Will I lose my progress when I switch?

Most people will continue to see weight loss after switching, but there might be some initial adjustment as your body adjusts to this new medication. Weightloss may temporarily plateau in the first few weeks, but is considered to be completely normal.

Will I start Wegovy at the same strength I was on Mounjaro?

At SheMed, we recommend starting Wegovy at an equivalent dose to Mounjaro (unless preferred otherwise by you). While the dosage may not look the same in numbers, the strength is similar and that’s exactly how it should be.

What is 1mg Wegovy equivalent to in Mounjaro?

There is no exact equivalent, but in practice, 1mg Wegovy is most commonly used as a starting or step-up dose for people coming from Mounjaro doses in the 7.5mg to 10mg range. Your prescriber will confirm the right starting point for you.

What is 2.4mg Wegovy equivalent to in Mounjaro?

2.4mg is the highest Wegovy dose and is broadly similar to the upper end of Mounjaro’s range (12.5mg to 15mg) in efficacy outcomes. These, however, are not equivalent in nature.
