Foundayo (Orforglipron) just got its MHRA approval on the 10th of August, 2026, and now women in the UK have four licensed GLP-1 weight loss options: two weekly injections (Wegovy and Mounjaro) and two daily tablets (the Wegovy pill and Orforglipron, brand name Foundayo).

All of them are prescription-only, all work through the same fundamental mechanism, and all have meaningful clinical trial data behind them. The differences are how they work at a chemistry level, how you take them, and what the average weight loss figures look like across different trials.

GLP-1 tablets and injections both work by mimicking GLP-1, a hormone in your gut that tells your brain you are full, slows how quickly food leaves your stomach, and regulates blood sugar. The difference is not the destination but the path.

What Makes a GLP-1 Medicine a Tablet Rather Than an Injection

Initially, GLP-1 medications were only available as injections. This is because they were made up of peptide molecules that if swallowed, get broken down by stomach acid, leaving no impact. So, for them to enter your bloodstream from a tablet would require either a bypass mechanism or building a molecule that doesn't have the problem in the first place.

The Wegovy pill uses the first option, a compound called SNAC, that creates a temporary window in the stomach that allows semaglutide to pass through the stomach lining. This is why it must be taken on an empty stomach with a precise amount of water, followed by a 30-minute fast. The absorption window is narrow, precise, and food-sensitive.

The makers of Orforglipron (Foundayo), Eli Lilly, went for the other option. They built a small non-peptide molecule, that survives stomach acid on its own without any special carrier. It absorbs reliably whether you have eaten or not, at any time of day, with any amount of water.

Foundayo (Orforglipron) vs the Wegovy Pill

This section is important for women who want a needle-free option. Both are once-daily oral GLP-1 medicines. Both are now MHRA-licensed. The differences are real and worth understanding.

The Fasting Routine: Orforglipron's Biggest Practical Advantage

The Wegovy pill comes with a set of strict instructions about how it’s supposed to be taken. An empty stomach, first thing in the morning, with no more than 120ml of plain still water, followed by a 30-minute wait before you eat, drink anything else, including any other oral medications.

Orforglipron doesn’t come with any particular restrictions. You take it once a day, at whatever time suits you, with any amount of water and any food. You could take it with your morning coffee, at lunch, or before bed.

If you found the Wegovy pill's routine difficult to maintain consistently, this is a genuine practical advantage. And in the end, consistency is what determines your results, not trial averages.  


The thing nobody tells you about the Wegovy pill: it works brilliantly when you do the routine right, but the 30-minute fasting window is the one thing that trips women up most often. School runs, early meetings, toddlers who need breakfast immediately, not having the stomach for a tablet first thing when nauseous.

Orforglipron simply removes that friction entirely. Take it whenever. That’s not a small thing when you’re doing this every day for a year.

 

Weight Loss Results Compared: What the Data Shows

In non-diabetes populations: the Wegovy pill produced 13.6% average weight loss at 64 weeks in the OASIS 4 trial. Orforglipron produced 12.4% average weight loss at 72 weeks in ATTAIN-1. An important thing to note here is that while the results are similar, they are from different trials with different populations and durations, so a direct comparison should be done carefully.

In type 2 diabetes populations (where we’ve got a direct head-to-head comparison): ACHIEVE-3 (published in The Lancet, February 2026) was the first Phase 3 trial directly comparing Orforglipron and oral semaglutide. At the highest doses, Orforglipron (36mg) produced 9.2% body weight reduction versus 5.3% with oral semaglutide (14mg). A 73.6% higher relative weight loss. HbA1c reduction was also greater: 2.2% with Orforglipron vs 1.4% with oral semaglutide.

Before you reach conclusions, here’s an important note: the ACHIEVE-3 trial compared oral semaglutide’s standard-dose (up to 14mg, the diabetes-licensed dose), not the higher-dose Wegovy pill formulation used for weight management (25mg). Hence, even this head-to-head comparison may not be conclusive.

 

Orforglipron Will Suit your Better Over the Wegovy Pill If -

  • You’re on levothyroxine: thyroid medication typically requires 30 to 60 minutes before food, already making mornings complicated. Adding the Wegovy pill's fasting window on top can get logistically difficult to maintain. Orforglipron sidesteps this entirely.
  • Women with PCOS or insulin resistance: both medications improve insulin sensitivity, but orforglipron's simpler routine can mean better adherence, which, in the end, determines the outcome.
  • Women who repeatedly miss or take the Wegovy pill incorrectly: Irrespective of the medicine, consistency is the key to effectiveness. If the fasting routine is the thing standing between you and consistent dosing, Orforglipron is a genuinely better fit.

Foundayo (Orforglipron) vs Mounjaro (Tirzepatide)

This comparison between an oral tablet and a weekly injection is not just about weight loss numbers but about what trade-offs you are willing to make.

Weight Loss Results: The Efficacy Gap

Mounjaro currently produces the highest average weight loss of any licensed weight management medicine in the UK with an average body weight reduction of 22.5% at 72 weeks at the 15mg dose.

In a head-to-head comparison with the Wegovy injection, Mounjaro produced 20.2% average weight loss vs Wegovy’s 13.7% at 72 weeks. This confirmed that the advantage persists head-to-head.

Orforglipron (Foundayo), on the other hand, produced a 12.4% average weight loss with a 36mg dose at 72 weeks. Substantially below Mounjaro on average results.

So, if maximum average weight loss is your primary goal and you are able to self-inject weekly, Mounjaro remains the most effective option available for you.

Why Injections Currently Outperform Oral Options

Pharmacokinetics. When you inject a GLP-1 medicine subcutaneously (just under your skin), it enters your bloodstream with almost full bioavailability. With any oral GLP-1, even with optimised delivery technology, some of the drug is processed before it reaches your blood circulation system. Orforglipron's non-peptide design is the best oral delivery yet developed, but it cannot beat the absorption percentage of an injection.

This doesn’t automatically mean that you should go for injections. If 12 to 13% weight loss, without any injections, fits your goals and suits your lifestyle better, the additional results from a weekly injection may not be worth it. The right medicine is the one you can take consistently,

Women on levothyroxine should mention this to their prescribing clinician when discussing options.
